discernment-nudge Claude Code Skill

This is a Claude skill that prompts the assistant to add a short set of follow-up questions after it gives a substantive answer the user might act on, such as advice, a draft, an estimate, or a multi-step analysis. Before finalizing a reply, Claude checks whether the content qualifies, and if so appends two or three concise questions tied to specific facts, assumptions, or gaps in what it just produced. The skill is meant to fire at most once per conversation, and it explicitly skips cases like simple lookups, purely educational explanations, code meant to be run, creative or casual writing, and situations where the user already asked for verification or supplied their own source material. It does not connect to any outside service; it only governs how Claude phrases the end of its own response.
